Choose an auspicious day after checking the Hindu Calendar preferably a Wednesday or Thursday. 
Have a bath & wear clean clothes. 
Sit in a comfortable sitting posture while facing East. 
Perform Pranayama Anolom Vilom (Alternating nostril breaths) 10 times & meditate to purify your mind and thoughts. 
Place the Maha Sudharshan Yantra in front of you on a Puja Thali. 
Light a Diya Lamp and Offer Namaskar.

Using Kusha Grass or a Spoon sprinkle Gangajal on yourself and your surroundings while chanting Pavitrikaran Mantra:  
"Om Apavitrah Pavitro Va Sarva Vastan Gatopi Va 
Yah Smaret Pundari Kaksham Sa Bahya Bhyantarah Shuchih"

Translation: 
May all things unholy become holy, may all lower tendencies depart, just as soon as we transcend may within and without be purified!

Chant Maha Sudharshan Mantra: Om Gum Ganapataye Namaha (11 times) 
Chant Shiva Mantra: Om Namah Shivaye (11 times) 
Chant Vishnu Mantra: Om Namo Bhagavate Vasudevaya (11 times) 
Remember all the 9 planets Sun (Surya), Moon (Chandra), Mars (Mangal), Mercury (Budha), Jupiter (Brihaspati), Venus (Shukra), Saturn (Shani), Rahu & Ketu. Remember all the 5 elements Earth, Water, Fire, Air & Ether. 
Perform Abhishek (Ritualistic Bathing) of the Maha Sudharshan Yantra using Water, Milk, Honey, Panchamrit & Sugar while chanting “Om Namo Narayanaya”. 
Wash the Maha Sudharshan Yantra with Water & Wipe it with a Clean Cloth. 
Offer Kum while chanting Maha Sudharshan Yantra Beej Mantra: “Om Namo Narayanaya '' after each recitation. You may chant this Mantra 9/21/51 or 108 times. 
Perform Panchupchara Puja.

Some may not be able to perform the puja as mentioned above, in this case one can follow the simple puja procedure mentioned below. Note that any form of worship done with a positive intention will bless the devotees.

One can worship Maha Sudharshan Yantra using the technique mentioned below. 
Choose an auspicious day after checking the Hindu Calendar preferably a Wednesday or Thursday. 
Have a bath & wear clean clothes. 
Sit in a comfortable sitting posture while facing East. 
Perform Pranayama Anolom Vilom (Alternating nostril breaths) 10 times & meditate to purify your mind and thoughts. 
Place the Maha Sudharshan Yantra in front of you on a Puja Thali. 
Light a Diya Lamp and Offer Namaskar. 
Sprinkle water on yourself and the Maha Sudharshan Yantra. 
Apply Sandal Paste to the Bindu of the Maha Sudharshan Yantra. 
Offer Kum to the Maha Sudharshan Yantra. 
Offer Fruits or Sweets to the Maha Sudharshan Yantra. 
Light a Dhoop Incense and Offer that to the Maha Sudharshan Yantra. 
Chant the Beej Mantra “Om Namo Narayanaya” 9/21/51 or 108 times.

How to Meditate on Maha Sudharshan Yantra? (Visualization Technique)

Meditating on the Maha Sudharshan Yantra is the most powerful way to achieve union with the diagram. Achieving the union is said to reveal all secrets of the world and one is supposed to gain access to knowledge of higher realms. In an oversimplified sense one must recreate the Maha Sudharshan Yantra diagram accurately in the mind and visualize it. This visualization technique may sound simple but is quite tough to achieve, one must spend years of sadhana just to create an accurate image of the diagram in their head.

One can start by sitting in a comfortable posture facing east with an accurate image of Maha Sudharshan Yantra at their Eye Level.

Perform Pranayama Anolom Vilom (Alternating nostril breaths) 10 times & meditate to purify your mind and thoughts.

Start by focusing on the Centre Point Bindu with your gaze going inward to outward from the Bindu to the 1st Layer of the Shaktona or two interlocked triangles to 2nd Layer of the Circle to the 3rd Layer of the 8 Petals to the 4th Layer of the Circle to the 5th Layer of the 16 Petals to the 6th Layer of the Circle to the 7th Layer of the Discus encircled by first focusing your gaze in the little circles and then in the 108 indented edges pointing outwardly in all directions as a wheel. These are the steps or Avaran of Maha Sudharshan yantra.

Then your gaze should go from the Outermost Gate back to the Bindu.

Try to avoid blinking your eyes during this process. You can do this for multiple rounds for 30 minutes.

After you can focus with intent for at least 30 minutes (the stamina to just focus may take quite some time to develop) close your eyes and try to recreate the image of the Maha Sudharshan Yantra Diagram accurately.

Once you can create and sustain an accurate image of Maha Sudharshan Yantra in your mind try to focus one by one on the steps mentioned above and go from Bindu to Outermost gate & Back to Bindu however all this needs to be done in your mind in meditation with your eyes closed.

This visualization technique takes many years to master and bless the Sadhak with material success as well as spiritual augmentation.

Where to install Maha Sudharshan Yantra?

Maha Sudharshan Yantra will energize the area where it is placed, it can be installed anywhere in your home, office, factory, dwelling. Ideally it should be placed in the East Corner of a room facing West. Yantras get energized by the Energy of the Sun.
