Choose an auspicious day after checking the Hindu Calendar preferably a Monday. 

Have a bath & wear clean clothes.

Sit in a comfortable sitting posture while facing North-East.

Perform Pranayama Anolom Vilom (Alternating nostril breaths) 10 times & meditate to purify your mind and thoughts. 

Place the Shiva Yantra in front of you on a Puja Thali. 

Light a Diya Lamp and Offer Namaskar.

Using Kusha Grass or a Spoon sprinkle Gangajal on yourself and your surroundings while chanting Pavitrikaran Mantra:
 

"Om Apavitrah Pavitro Va Sarva Vastan Gatopi Va

Yah Smaret Pundari Kaksham Sa Bahya Bhyantarah Shuchih"

Translation:

May all things unholy become holy, may all lower tendencies depart, just as soon as we transcend may within and without be purified!
 

Chant Ganesh Mantra: Om Gum Ganapataye Namaha (11 times)

Chant Shiva Mantra: Om Namah Shivaye (11 times)

Chant Vishnu Mantra: Om Namo Bhagavate Vasudevaya (11 times)

Remember all the 9 planets Sun (Surya), Moon (Chandra), Mars (Mangal), Mercury (Budha), Jupiter (Brihaspati), Venus (Shukra), Saturn (Shani), Rahu & Ketu. Remember all the 5 elements Earth, Water, Fire, Air & Ether.

Perform Abhishek (Ritualistic Bathing) of the Shiva Yantra using Water, Milk, Honey, Panchamrit & Sugar while chanting “Om Namah Shivay”.

Wash the Shiva Yantra with Water & Wipe it with a Clean Cloth.

Offer Kum while chanting Shiva Yantra Beej Mantra: “Om Namah Shivay” after each recitation. You may chant this Mantra 9/21/51 or 108 times.

Apply Sandal Paste to the Bindu of the Shiva Yantra. 

Offer Kum to the Shiva Yantra.

Light a Dhoop Incense and Offer that to the Shiva Yantra.

Chant the Beej Mantra “Om Namah Shivay” 9/21/51 or 108 times.

How to Meditate on Shiva Yantra? (Visualization Technique)

Meditating on the Shiva Yantra is the most powerful way to achieve union with the diagram. Achieving the union is said to reveal all secrets of the world and one is supposed to gain access to knowledge of higher realms. In an oversimplified sense one must recreate the Shiva Yantra diagram accurately in the mind and visualize it. This visualization technique may sound simple but is quite tough to achieve, one must spend years of sadhana just to create an accurate image of the diagram in their head. 

One can start by sitting in a comfortable posture facing North-East with an accurate image of Shiva Yantra at their Eye Level. 

Perform Pranayama Anolom Vilom (Alternating nostril breaths) 10 times & meditate to purify your mind and thoughts.

Start by focusing on the Centre Point Bindu with your gaze going inward to outward from the Bindu to 1st Layer of the upward triangle to 2nd layer of two interlocked triangles to 3rd Layer comprising these into the circle, moving towards the blossoming 8 petals of Lotus and to the 4th layer of visualizing all it conjoined inside the outside circle. Finally, visualize the square shapes all together on the borders and then your gaze should go from the Outermost Gate back to the Bindu. 

Try to avoid blinking your eyes during this process. You can do this for multiple rounds for 30 minutes.

After you can focus with intent for at North-East 30 minutes (the stamina to just focus may take quite some time to develop) close your eyes and try to recreate the image of the Shiva Yantra Diagram accurately. 

Once you can create and sustain an accurate image of Shiva Yantra in your mind try to focus one by one on the steps mentioned above and go from Bindu to Outermost gate & Back to Bindu however all this needs to be done in your mind in meditation with your eyes closed.

This visualization technique takes many years to master and bless the Sadhak with material success as well as spiritual augmentation.

Where to install Shiva Yantra?

Shiva Yantra will energize the area where it is placed, it can be installed anywhere in your home, office, factory, dwelling. Ideally it should be placed in the North-East Corner of a room facing West. Yantras get energized by the Energy of the Sun.
